A speech therapy invoice is usually a superbill: the family pays you and then asks their plan to reimburse them. The plan reads codes, not sentences, and returns the claim for what is missing rather than for what it costs — so the identifiers matter more here than the wording of any line.

What to put on a speech therapy invoice

  • Your NPI, licence number and the practice tax ID
  • Each session by date, with its CPT code
  • Whether the code is untimed or billed in units, since SLP codes are mostly untimed
  • The diagnosis code, which nearly every plan requires to reimburse
  • Place of service — clinic, home, school or telehealth
  • The amount already paid, since a superbill documents a payment the family has made

Questions

Is 92507 billed by time?

No. Individual speech, language, voice and communication treatment is service-based: one unit per session however long the session ran, and it may only be billed once a day. That is unlike the physical and occupational therapy codes families often compare it to, which are timed.

What makes a superbill get rejected?

Almost always a missing identifier rather than a disputed price — no NPI, no diagnosis code, or a date that does not match the plan's record. The fee itself is rarely the problem, because the family has already paid it.

Do I need separate codes for evaluation and treatment?

Yes. An evaluation is its own code and its own reimbursement; billing an assessment session as treatment is a common cause of a clawback when the plan reviews the claim months later.
