Training is usually sold in blocks and consumed one session at a time, so the invoice has to do something a receipt does not: show how many sessions were used and how many remain. Most disputes between trainers and clients are arithmetic disputes, and they end the moment the count is written down somewhere both people can see.

What to put on a personal training invoice

  • The sessions delivered in the period, counted
  • Any package purchased, with sessions remaining after this invoice
  • Single sessions at the single-session rate, which is rarely the package rate
  • Programming or nutrition work, which is time spent away from the client
  • Late cancellations charged, with the policy stated in a line rather than assumed
  • The location, where you train at more than one and travel differs

Questions

How do I bill a late cancellation?

As a line of its own, at the rate your policy states, with the date. Deducting a session from the client's block without saying so is the version that ends the working relationship.

Should package sessions be shown even when they are prepaid?

Yes. An invoice showing five sessions used and seven remaining is worth more to the client than a receipt showing a payment they made a month ago and have already forgotten.
