Chimney Sweep Invoice Template
Sweeping, inspection level, creosote condition, caps and liners. Free PDF.
A chimney invoice is a safety record as much as a bill. What the sweep found — creosote stage, a cracked flue tile, a missing cap — is the part that matters six months later when the homeowner is deciding whether to light a fire, and it is what an insurer asks for after a chimney fire.

What to put on a chimney sweep invoice
- The appliance and flue serviced, where the property has more than one
- The inspection level performed, since a visual check and a camera scan are different products
- Creosote condition found, by stage, and how much was removed
- Any defect found, described plainly, with whether it makes the flue unsafe to use
- Caps, dampers, liners and masonry work as their own lines
- Your certification, which is what an insurer looks for on the document
Questions
Why does the inspection level matter?
Because they answer different questions. A visual inspection covers the readily accessible parts; a camera scan of the flue interior is what finds cracked tiles and gaps behind them. A homeowner buying a house, or one who has had a chimney fire, needs the second and should not be sold the first.
Should I write down creosote stage?
Yes. Stage three glazed creosote is a fire hazard and a reason to stop using the appliance, and the invoice is the only place that assessment survives. It also justifies the sweep frequency you are recommending.
What if I find the flue unsafe?
Say so on the invoice in plain words, not only in conversation. It is the document the homeowner keeps, and if there is later a fire it is the record that the condition was reported.
