What to put on a market research invoice

The project name and the methodology — survey, focus group, in-depth interviews
Design and questionnaire development, which happens before any fieldwork
Fieldwork by completed interview, with the achieved sample size
Respondent incentives at cost, since this is the respondents' money not yours
Panel or recruitment costs, and any screening over-recruitment
Analysis and reporting, separately from fieldwork

Questions

Why show incentives separately?

Because they are pass-through money going to respondents. On a large sample they can exceed the agency fee, and a client who cannot see the split has no way to judge what the research itself cost.

How should an under-achieved sample be billed?

By what was completed, with the shortfall named. Fieldwork sometimes fails to reach quota on a hard-to-find audience, and an invoice that quietly bills the quoted sample regardless is the one that ends the relationship.
