What to put on a tutoring invoice

The student's name, since the payer is often not the person taught
Lesson dates and durations, rather than a single monthly total
The subject or exam being prepared for
Materials, past papers or exam entry fees passed on
Missed lessons and how they were treated under your policy
Whether lessons are online or in person, where the rate differs

Questions

Should I invoice per lesson or monthly?

Monthly is easier for both sides, provided the lessons are listed inside it. What a parent objects to is not the total but being unable to check it.

How do I handle a missed lesson?

State the policy on the invoice and apply it visibly. A missed lesson charged without explanation reads as an error; the same charge with 'under 24 hours' notice' beside it reads as the agreement working.
