Doula work used to be paid one way — privately, often as a package with a deposit. It is no longer only that: more than half of states now reimburse doula services through Medicaid, and a doula billing a state programme needs a very different document from one billing a family. This template does both, because most doulas now do both.

What to put on a doula invoice
- The client's name and the expected or actual date of birth
- Prenatal visits counted, since packages are usually priced around a number of them
- Labour and delivery support as its own line, whatever the hours turned out to be
- Postpartum visits, counted, with their length
- Whether this is private pay or a Medicaid claim, since they need different identifiers
- For a Medicaid claim: your enrolment or provider number and the authorising details your state requires
Questions
Is doula care covered by Medicaid?
In a growing number of states, yes. As of March 2026, twenty-six states and the District of Columbia reimburse doula services through Medicaid — fourteen more than two years earlier — with several further states in the process of implementing it. Whether you can bill depends on your state's programme and on being enrolled with it.
How is Medicaid doula work priced?
By your state's fee schedule rather than by your private rate, and the two are usually different. Labour and delivery support across states ranges roughly from a few hundred dollars to around fifteen hundred, with prenatal and postpartum visits paid per visit. Bill the schedule, not the package.
Should a private-pay package show the individual visits?
Yes. A package price with nothing underneath it is hard for a family to value and impossible to prorate if the birth goes differently than planned — an early delivery, a transfer, a birth you missed. Listing the visits is what makes a fair adjustment possible.
